One way of doing this is using your Paint program that is part of Windows operating system and usually found in your Accessories folder (START >> ALL PROGRAMS >> ACCESSORIES >> PAINT).

Scan the page and save it to your computer or to your desktop. Then open it with Paint. In Paint you can fill it in, save it back to your computer. Then print it out from there. Don't try to print it out while you are in Paint as it will be quite large to work with in Paint but it will revert to a normal size when you save it. By having saved it to the computer in a folder you can re-use the scanned form again and type in other info or others can type their info in the form.

If you have the paid Acrobat program (Adobe Acrobat Reader) I believe you can also scan and save it as a .pdf file, then use Acrobat to open it and type in the spaces as needed and print it out. You may also be able to scan and save it as a Word document from the scanner and then after opening the file with Word, fill in the info as needed. If your Scanner will let you use another word processor format to save it as you don't have MS Word on the computer, then you may be able to do it that way as well. Some pdf programs also allow you to open a pdf document, fill in the form and then save it as another pdf form which you can print from the program. There are also some on-line sites where you can upload a .pdf file and they will convert it to a word processor document format so you can fill in the spaces on it.....

Just remember that changing from a .pdf format to a word processor format might change the appearance of the scanned document, and I don't know how critical that might be to you. Scanning and saving in the .pdf format pretty much keeps a form in the original appearance.....

But, since paint is included with the PC just using that free program process is the cheapest and simplest way of accomplishing what you want and not having to do a lot of conversions and so forth.
